Đặt vấn đề: Ung thư vú ở phụ nữ trẻ (< 45 tuổi) thường có đặc điểm sinh học ác tính và tiên lượng kém. Trong số các dấu ấn phân tử, p53 và Ki-67 được quan tâm vì có liên quan đến tiến triển bệnh và đáp ứng điều trị; Mục tiêu nghiên cứu: đánh giá mối liên quan giữa p53 và Ki-67 với một số đặc điểm giải phẫu bệnh trong ung thư vú ở phụ nữ trẻ.
Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang trên 51 bệnh nhân ung thư biểu mô tuyến vú xâm nhập, tuổi < 45, được chẩn đoán và điều trị tại Bệnh viện Trường Đại học Y Dược Huế giai đoạn 01/2023–06/2025. Các mẫu mô được nhuộm HE và hóa mô miễn dịch (p53, Ki-67, ER, PR, Her2). Phân tích thống kê sử dụng phép Chi-square với ngưỡng ý nghĩa p < 0,05.
Kết quả: Tuổi trung bình của bệnh nhân là 38,5; nhóm tuổi 35–40 chiếm tỷ lệ cao nhất (56,9%). Thể mô bệnh học chủ yếu là ung thư biểu mô ống xâm nhập không đặc hiệu (NST, 76,5%). Giai đoạn II–III chiếm gần 80%, trong đó 58,8% có di căn hạch nách. Tỷ lệ p53 dương tính là 41,2%, trong khi Ki-67 cao (≥ 14%) chiếm 82,5%. P53 dương tính có xu hướng tăng ở độ mô học III (62,5%), nhưng chưa đạt ý nghĩa thống kê (p = 0,057). Ki-67 cao có liên quan có ý nghĩa với độ mô học (p < 0,05), nhưng không có mối liên quan với kích thước u, hạch hay giai đoạn bệnh. Đáng chú ý, có mối liên quan có ý nghĩa giữa p53 và Ki-67: 90,5% trường hợp p53 dương tính kèm Ki-67 cao (p < 0,05).
Kết luận: Biểu hiện p53 và Ki-67 cao là đặc điểm thường gặp trong ung thư vú ở phụ nữ trẻ, phản ánh sự tăng sinh mạnh và độ ác tính cao của ung thư. Sự phối hợp p53 dương tính và Ki-67 cao góp phần giải thích tiên lượng xấu ở nhóm bệnh nhân này, đồng thời gợi ý giá trị của việc sử dụng kết hợp hai dấu ấn này trong phân tầng nguy cơ và định hướng điều trị cá thể hóa.
