Đặt vấn đề: Xơ cứng bì toàn thể (XCBTT) là một bệnh lý mô liên kết hệ thống có tổn thương đa cơ quan. Bệnh phổi kẽ (BPK) là một trong những tổn thương cơ quan làm tăng biến chứng và tử vong ở bệnh nhân XCBTT. Nghiên cứu này nhằm mục tiêu đánh giá mối liên quan giữa một số yếu tố lâm sàng, cận lâm sàng với dung tích sống gắng sức (%FVC) trên bệnh nhân XCBTT có BPK.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu cắt ngang này được tiến hành nhằm đánh giá các đặc điểm lâm sàng, cận lâm sàng của bệnh phổi kẽ (BPK) liên quan đến xơ cứng bì toàn thể (XCBTT) và mối liên quan giữa các yếu tố này với dung tích sống gắng sức (%FVC).
Kết quả: Nghiên cứu thu nhận tổng cộng 106 bệnh nhân XCBTT có BPK, được chẩn đoán dựa trên tiêu chuẩn ACR/EULAR 2013 và hình ảnh cắt lớp vi tính lồng ngực độ phân giải cao (HRCT). Dân số nghiên cứu chủ yếu là nữ giới (91,5%), với độ tuổi trung bình là 51,3 ± 10,3 tuổi và thời gian mắc bệnh trung vị là 5 năm. Bệnh nhân trong nghiên cứu có tình trạng suy giảm chức năng phổi mức độ trung bình, với %FVC dự đoán trung bình là 60,84%. Về kiểu hình tổn thương, viêm phổi kẽ không đặc hiệu (NSIP) là kiểu hình thường gặp nhất (53,8%), kế đến là viêm phổi kẽ thông thường (UIP) chiếm 41,5%. Phân tích đơn biến cho thấy %FVC có liên quan nghịch với nồng độ LogKL6, áp lực động mạch phổi thì tâm thu (PAPs), và số lượng tiểu cầu (PLT). %FVC dự đoán của kiểu tổn thương UIP cũng thấp hơn có ý nghĩa thống kê so với kiểu hình NSIP. Sau phân tích hồi quy tuyến tính đa biến, chỉ còn hai yếu tố có liên quan có ý nghĩa thống kê với %FVC dự đoán là LogKL6 (p < 0,001) và PAPs (p = 0,012). Nghiên cứu nhấn mạnh tầm quan trọng của việc tầm soát BPK do các triệu chứng lâm sàng (như ho khan, khó thở) chỉ gặp trong khoảng một nửa số bệnh nhân.
Kết luận: Bệnh phổi kẽ là một tổn thương cơ quan quan trọng trong XCBTT. Cần tầm soát BPK ở mọi bệnh nhân XCBTT ngay cả khi không có triệu chứng lâm sàng. KL-6 là một yếu tố lâm sàng quan trọng có liên quan với BPK ở bệnh nhân XCBTT.
