Outcomes of single dose prophylactic antibiotics in laparoscopy for acute uncomplicated appendicitis

Abstract

Background-Objectives: The administration of antibiotics in patients undergoing laparoscopic appendectomy  remains a matter of concern. This study aimed to evaluate the outcomes of a single-dose prophylactic antibiotic  regimen in laparoscopic surgery for acute uncomplicated appendicitis.  

Materials and Methods: This prospective study included 414 patients with non-perforated appendicitis who  underwent laparoscopic appendectomy at Hue University of Medicine and Pharmacy Hospital from January  2022 to December 2023. Among them, 207 patients in Group 1 received a single dose of prophylactic antibiotics,  while 207 patients in Group 2 received postoperative antibiotics.  

Results: There were no statistically significant differences between the two groups in terms of demographic  and clinical characteristics. The mean operative time was 29.5 ± 6.1 minutes in Group 1 compared to 29.3 ± 6.0  minutes in Group 2 (p > 0.05). The time to bowel motility recovery after surgery was 12.8 ± 3.8 hours in Group 1  and 12.6 ± 2.4 hours in Group 2 (p > 0.05). Postoperative complications occurred in 5.3% of patients in Group 1  and 6.3% in Group 2 (p > 0.05). The duration of analgesic use, intravenous antibiotic treatment, and postoperative  hospitalization were significantly shorter in Group 1 (1.1 ± 0.4 days, 0.2 ± 1.1 days, and 2.5 ± 1.0 days, respectively)  compared to Group 2 (1.4 ± 0.5 days, 3.0 ± 1.0 days, and 3.5 ± 1.0 days, respectively) (p < 0.05). 

Conclusion: A single-dose prophylactic antibiotic regimen is sufficient for laparoscopic surgery in patients  with acute uncomplicated appendicitis.
