Abstract
Background: Anemia in rheumatoid arthritis (RA) is a condition associated with chronic inflammation. Early detection and treatment of anemia may contribute to improving patient outcomes. In RA, inflammation is characterized by elevated levels of serum inflammatory markers, notably C-reactive protein (CRP), and disease activity is commonly assessed using the Disease Activity Score 28 (DAS 28).
Objectives: 1. To determine serum CRP levels, DAS 28 CRP, DAS 28 ESR scores, and anemia status in
patients with RA; 2. To investigate the relationship between serum CRP levels, anemia status, and clinical and paraclinical indices in RA patients.
Methods: A cross-sectional descriptive study was conducted on 39 patients diagnosed with RA at Hue University of Medicine and Pharmacy Hospital.
Results: The median serum CRP level was elevated at 45.72 [12.15 - 91.14] mg/L. Two-thirds of patients had high disease activity (DAS 28 > 5.1). Anemia was observed in 51.3% of patients, with microcytic anemia accounting for 7.7% and normocytic anemia for 43.6%. Hypochromic anemia, based on mean corpuscular hemoglobin (MCH), was present in 28.2% of cases. CRP levels showed a strong positive correlation with first
hour erythrocyte sedimentation rate (ESR) (p = 0.001, r = 0.515) and swollen joint count (p < 0.001, r = 0.533), and a moderate positive correlation with tender joint count (p = 0.003, r = 0.463). A significant association was found between anemia and the number of painful joints in patients (p = 0.02).
Conclusion: Most RA patients exhibited elevated CRP levels, high disease activity, and more than 50% had anemia. There was a strong correlation between the DAS 28 CRP and DAS 28 ESR scores. The study found a relationship between anemia, CRP levels, and tender joint count. CRP was strongly correlated with first-hour ESR.
