Study of diabetic peripheral neuropathy by electromyography in patients with type 2 diabetes

Abstract

Background: Diabetic peripheral neuropathy is a major form of complication that arises in long-term diabetic patients. Electromyography is the gold standard for diagnosing diabetic peripheral neuropathy.
Objective: To investigate nerve conduction characteristics, prevalence of peripheral neuropathy by electromyography and to determine the relationship between Diabetic peripheral neuropathy and risk factors.
Subjects and methods: cross-sectional descriptive study on 215 type 2 diabetes patients being treated as inpatients at the Department of Endocrinology, Thu Duc Regional General Hospital. Using Nemus 2 electromyography machine.
Results: The rate of peripheral neuropathy complications according to electromyography was 56.7%. Peripheral neuropathy complications in patients with type 2 diabetes were associated with age (OR: 1.06; 95% CI: 1.03 - 1.09), duration of diabetes (OR: 1.15; 95% CI: 1.09 - 1.2), metformin treatment (OR: 2.28; 95% CI: 1.31 - 3.98), history of hypertension (OR: 3.22, 95% CI: 1.76 - 5.9), hypercholesterolemia (OR: 0.44; 95% CI: 0.25 - 0.77) and hypertriglyceridemia (OR: 0.38, 95% CI: 0.2 - 0.72); all with p < 0.05.
Conclusion: The rate of peripheral neuropathy in patients with type 2 diabetes is high (56.7%). Older age, longer diabetes duration, metformin treatment, history of hypertension, hypercholesterolemia and hypertriglyceridemia are factors associated with diabetic peripheral neuropathy in patients with type 2 diabetes. Age and duration of diabetes are two independent predictors of peripheral neuropathy in type 2 diabetes. As age increased by one year, the odds of developing diabetic peripheral neuropathy (DPN) increased by approximately 3% (OR: 1.03; 95% CI: 1.0 - 1.06; p = 0.043), while each additional year since the diagnosis of diabetes was associated with an approximately 12% (OR: 1.12; 95% CI: 1.05 - 1.72; p < 0.001)increase in the odds of developing DPN.
