Abstract
Objective: To determine the association between non-high-density lipoprotein cholesterol (non-HDL-C) levels and the severity of coronary artery disease (CAD), and to evaluate the value of non-HDL-C in predicting severe CAD in patients with obstructive ischemic heart disease (IHD).
Materials and methods: This cross-sectional study included 226 patients with obstructive IHD who underwent coronary computed tomography angiography. Obstructive IHD was defined as ≥ 50% stenosis in at least one major epicardial coronary artery. CAD severity was assessed using the Gensini score and patients were categorized into three groups according to tertiles: mild (≤ 32 points), moderate (> 32 and ≤ 56 points) and severe (> 56 points). Blood lipid profiles were measured within 24 hours of hospital admission.
Results: Non-HDL-C levels increased progressively across the three Gensini severity groups with statistically significant differences (p < 0.001). Spearman correlation analysis showed a moderate positive association between non-HDL-C and the Gensini score (rho = 0.426; p < 0.001), and non-HDL-C had the strongest correlation among lipid parameters. Multivariate logistic regression analysis showed that non-HDL-C was independently associated with moderate-to-severe CAD (odds ratio, OR = 1.376; 95% confidence interval [CI] 1.060 - 1.786; p = 0.002), together with diabetes mellitus (OR = 3.032; 95% CI: 1.552 - 5.922; p = 0.001) and typical angina (OR = 2.402; 95% CI: 1.241 - 4.651; p = 0.009). Receiver operating characteristic analysis showed that non-HDL-C predicted severe CAD with an area under the curve (AUC) of 0.719 (95% CI: 0.653 - 0.785; p < 0.001) at a cutoff > 2.62 mmol/L (sensitivity 91.8%, specificity 43.8%). For moderate-to-severe CAD, the AUC of non-HDL-C was 0.685 (95% CI: 0.608 - 0.761; p < 0.001) with a cutoff > 2.47 mmol/L, sensitivity 83.8%, specificity 51.3%.
Conclusions: Non-HDL-C shows a moderate positive correlation with CAD severity in patients with obstructive IHD and is independently associated with moderate-to-severe CAD. Non-HDL-C may help support the assessment of CAD severity in this population
