Evaluation of treatment outcomes and quality of life in patients with tibial plateau fractures undergoing bone fixation using the 3-column concept

Abstract

Background: The three-column classification based on CT images has been introduced to evaluate tibial plateau fractures and classify them more accurately, thereby helping to plan specific treatment for patients.

Objectives: to evaluate the treatment results and draw experiences from the application of the three-column concept in tibial plateau fracture surgery.

Materials and methods: This study included 40 patients diagnosed with tibial plateau fractures treated with internal fixation surgery using the 3-column concept at Da Nang Hospital from March 2022 to July 2023.

Results: 87.5% of patients were of working age, mostly due to traffic accidents (92.5%). Single-column fractures (47.5%) were the most common fracture pattern, while lateral-column fractures accounted for the highest proportion (85.0%), posterior-column fractures accounted for 40.0%. The anterolateral approach was mainly performed (92.5%), the posterior approach accounted for 35.0%. The average surgical time was 81.2 ± 13.5 minutes, the more columns involved in the fracture, the longer the average surgical time, this difference was statistically significant (p<0.001). The rate of satisfactory reduction was 80.0%. 97.5% of patients had primary wound healing without infection. The results of postoperative treatment according to the Larson–Bostman criteria were up to 82.5% of patients achieved very good and good results. At 6 months postoperatively, up to 95.0% of patients achieved excellent and good results according to the Rasmussen scale and 100% of patients had radiographic bone union. 80.0% of patients achieved satisfactory quality-of-life outcomes at 6 months postoperatively. Conclusion: The application of the 3-column concept in the treatment of tibial plateau fractures may assist in injury assessment and surgical planning, particularly in complex fracture patterns. The initial study results demonstrated promising short-term outcomes in terms of functional recovery and postoperative quality of life.
