Abstract
Background: Hashimoto’s thyroiditis (HT) is the leading cause of acquired hypothyroidism in children. Characterization and classification of thyroid function, as well as identification of associated factors, are essential for developing appropriate management strategies.
Objectives: 1) Describe the characteristics and classify thyroid function in children with Hashimoto’s thyroiditis; 2) Investigate factors associated with thyroid function classification in children with Hashimoto’s thyroiditis.
Materials and methods: A cross-sectional descriptive study was conducted in 32 pediatric patients diagnosed with Hashimoto’s thyroiditis at Hue University of Medicine and Pharmacy Hospital from June 2024 to April 2025.
Results: Regarding the clinical characteristics of Hashimoto’s thyroiditis, euthyroid patients predominantly presented with goiter and had no systemic symptoms. Thyroid ultrasonography showed that the two most characteristic findings were heterogeneous thyroid parenchyma (87,5%) and increased vascularity (71,9%). Thyroid volume exceeded the World Health Organization (WHO) reference standards in 31,3% of patients. Regarding thyroid function, the longest disease duration was observed in the hypothyroid group (median, 11 months), followed by the hyperthyroid group (1,5 months) and the euthyroid group (1 month). Disease duration was significantly associated with thyroid function classification (p < 0.05). Asymptomatic patients predominated in the euthyroid group, whereas prominent systemic symptoms were observed only in patients with thyroid dysfunction. Thyroid volume exceeding the WHO reference standards occurred predominantly in the hypothyroid and hyperthyroid groups.
Conclusion: The clinical manifestations of Hashimoto’s thyroiditis in children are nonspecific. Measurement of thyroid autoantibodies should be considered in children presenting with goiter to establish a definitive diagnosis. Thyroid function may vary among patients, ranging from euthyroidism to hypothyroidism or hyperthyroidism. Regular monitoring every 4-6 weeks is recommended to allow timely therapeutic intervention.
