Abstract
Objectives: To evaluate the relationship of serum fibrinogen-albumin ratio with major cardiovascular events and TIMI score in patients with ST-elevation myocardial infarction underwent percutaneous coronary intervention.
Methods: Cross-sectional descriptive study on 131 patients diagnosed with ST elevation myocardio infarction who underwent PCI at Binh Dinh General Hospital from July 2022 to August 2023 and monitoring patients for 06 months after discharge.
Results: Samples of 131 patients completed the study. The average age is 64.1 ± 11 years old, men account for 74%. The mean FAR is 9.35 ± 4.0. The FAR was defined as the concentration ratio of fibrinogen (mg/dl) to albumin (mg/dl) multiplied by 100. The endpoint was the incidence of major adverse cardiovascular events (MACEs), including cardiovascular mortality, myocardio reinfarction, celebral stroke and hospital heart failure. After discharge, during 01 month and 06 months there are 15 MACEs (11.45%) and 25 MACEs (19.1%) respectively. Among 25 patients in MACEs group, they were less male and more older, hightly of serum fibrinogen concentration, heart rate, FAR and TIMI score than patients in group without MACEs (p<0.05). Admission FAR was positively correlated with TIMI score (r=0.21, p=0.02). To predict MACEs within 01 month, the cut-off point FAR=12.61 has a sensitivity of 66.7% and specificity of 86.3%, area under the curve AUC=0.73. To predict MACEs within 06 months, the cut-off point FAR = 14.95 has a sensitivity of 32% and a specificity of 95.3%, AUC=0.67. In the univariate logistic regression analysis, high FAR was associated with MACEs during 01 month (OR: 11.89 CI 95% [3.41-41.4] p=0.001) and 06 months (OR 4.02 CI 95% [1.48-10.96] p=0.006). In particular, high FAR (> 14.95) was the only factor independently associated with MACEs within 06 months in the multivariate logistic regression analysis (p=0.039).
Conclusions: Hight FAR associated with short-term and mid-term prognosis of MACEs and positively correlate with TIMI score in patients with ST-elevation myocardial infarction undergoing percutaneous coronary intervention
