Early Clinical Outcomes of the Szabo T-Stenting Technique in Complex Coronary Bifurcation Interventions

Abstract

Background: Percutaneous coronary intervention (PCI) for complex bifurcation lesions requiring two stents remains a challenge for interventional cardiologists due to low success rates and suboptimal outcomes. Current two-stent intervention techniques have limitations such as multiple layers of stent overlap at the bifurcation site or incomplete coverage of the side branch ostium due to inaccurate placement. We describe an improved stenting technique, the Szabo T-stent technique, which combines T-stenting with the Szabo technique to accurately position the side branch stent at its ostium, ensuring complete coverage of the side branch ostium and minimizing stent overlap at the bifurcation angle and in the main vessel..

Objective: The study aims to evaluate the efficacy and safety of the Szabo T-stenting technique in the treatment of coronary artery bifurcation lesions.

Subjectives and method: 11 patients with coronary artery bifurcation lesions were treated using the Szabo T-stenting technique at the Cardiology Center, Cho Ray Hospital. This is a case series study with longitudinal follow-up.

Result: The study recorded 11 patients with a mean age of 61.82 ± 9.36 years, of whom 72.72% were male. Among the patients, 81.81% underwent PCI due to acute coronary syndrome and 18.18% due to chronic coronary syndrome. The bifurcation lesion sites were LM/LAD/LCx in 45.45% of cases and LAD/D in 54.55%. Medina classification showed 81.81% with type 1,1,1 and 27.27% with type 0,1,1. The mean SYNTAX score was 22.82 ± 5.79. All cases (100%) were technically and procedurally successful. During the mean follow-up period of 4.82 ± 1.99 months, no cases of death, stent thrombosis, major adverse cardiac events, or hospital readmission due to chest pain were recorded.

Conclusion: PCI for bifurcation lesions remains challenging, and an optimal two-stent technique requires a balance between full lesion coverage and minimal stent overlap at the carina and main vessel. The Szabo T-stenting technique appears to be a feasible and safe two-stent strategy for complex coronary artery bifurcation lesions.
