Study on the serum CRP/Albumin ratio in myocardial infarction patients

Abstract

Objective: To investigate the serum CRP/Albumin Ratio (CAR) and its correlation with the extent of coronary artery damage according to the Gensini score, serum hs-Troponin T levels (hs-TnT), left ventricular ejection fraction (LVEF), and regional wall motion abnormalities in patients with myocardial infarction (MI).

Subjects and Methods: A cross-sectional descriptive study was conducted on 98 patients diagnosed with acute myocardial infarction at the Cardiovascular Emergency and Intervention Department, Hue Central Hospital, from March 6, 2024, to September 6, 2024. The average age was 66.53 ± 11.27 years, and the proportion of STEMI was 45.5% and NSTEMI was 54.5%. The mean serum CRP, albumin levels, and CAR were

9.93 ± 9.26 mg/L, 38.43 ± 4.09 g/L, and 0.27 ± 0.25 mg/g, respectively. The mean Gensini score was 49.72 ± 33.63. The optimal cutoff value for the CAR was 0.246 mg/g, with an area under the curve of 0.702, sensitivity of 51.8%, and specificity of 83.3%, predicting the extent of coronary artery damage according to the Gensini score. Patients with a CAR higher than the cutoff value had higher mean hs-TnT levels, lower mean LVEF, and a higher rate of regional wall motion abnormalities compared to those with a CAR lower than the cutoff value (p < 0.05). The mean CAR was also higher in groups with elevated hs-TnT levels, reduced LVEF, and the presence of regional wall motion abnormalities on echocardiography (p < 0.05). There was a positive correlation between the CAR and hs-TnT and a negative correlation between the CAR and LVEF.

Conclusion: A high CAR is associated with factors reflecting disease severity, such as elevated hs-TnT levels, reduced LVEF, the presence of regional wall motion abnormalities, and multivessel coronary artery disease. A CAR cutoff of ≥ 0.246 mg/g predicts the severity of coronary artery stenosis and myocardial injury, impaired cardiac function, and an increased risk of regional wall motion abnormalities in patients with myocardial infarction.
