Abstract
Background: Left-sided diverticulitis occurred less frequently than right-sided diverticulitis in Asian countries; however, it was associated with a higher rate of complications and a greater likelihood of requiring surgical intervention.
Objectives: To evaluate the clinical, paraclinical features, and treatment outcomes of patients with complicated colonic diverticulitis.
Subjects and methods: Prospective study, included 53 patients diagnosed with complicated colonic diverticulitis who were treated at the Department of Digestive Surgery, Hue University of Medicine and Pharmacy Hospital, from April 2023 to January 2026.
Results: The overall mean age of patients with complicated colonic diverticulitis was 44.9 years. Patients with right-sided colonic diverticulitis were younger, with a mean age of 39.0 years, compared with 67.2 years in those with left-sided. The male/female ratio was 2.7. All patients presented with abdominal pain. The mean white blood cell count was 12.2 K/µL. Overall, 71.7% of patients had leukocytosis. The mean CRP was 101.7 mg/L. Among the study population, 46 patients received conservative management, whereas 7 underwent surgical treatment. All patients who underwent surgery had left-sided colonic diverticulitis. Hartmann's procedure was the predominant approach. The mean length of hospital stay was 6.2 days for patients with right-sided colonic diverticulitis, compared with 15.8 days for those with left-sided disease. For surgical treatment patients, the postoperative complication rate was 57.1%, and the mortality rate was 14.3%.
Conclusions: Complicated colonic diverticulitis was more frequently observed in the right colon than in the left colon. Patients with right-sided colonic diverticulitis were younger than those with left-sided disease. Left-sided complicated colonic diverticulitis was associated with a higher incidence of peritonitis, longer hospital stay compared with right-sided
