Đặt vấn đề: Điều trị bệnh tim mạch chuyển hóa là quá trình dài hạn, đòi hỏi sự phối hợp chặt chẽ giữa kiến thức điều trị và hành vi tuân thủ. Tuy nhiên, dữ liệu về mối liên quan giữa hai yếu tố này tại khu vực miền Trung còn hạn chế.
Mục tiêu: Khảo sát kiến thức và đánh giá mức độ tuân thủ sử dụng thuốc trên bệnh nhân ngoại trú điều trị bệnh tim mạch chuyển hóa.
Đối tượng và phương pháp nghiên cứu: Mô tả cắt ngang trên 329 bệnh nhân ngoại trú điều trị bệnh tim mạch chuyển hóa tại Bệnh viện Trường Đại học Y Dược Huế từ 07/2024 đến 09/2024.
Kết quả: Tỷ lệ tuân thủ dùng thuốc đạt mức cao (90,9%), tuy nhiên kiến thức sử dụng thuốc mức tốt chỉ chiếm 58,7%. Rào cản chính dẫn đến quên thuốc không chủ ý, bao gồm quên do lịch trình bận rộn (43,5%) và gặp khó khăn khi nhớ lịch dùng (36,8%). Ngược lại, thái độ điều trị rất tích cực khi tỷ lệ tự ý ngưng thuốc (có chủ ý) do tác dụng phụ, cảm thấy khỏe hay khó khăn tài chính chiếm tỷ lệ rất thấp. Đáng chú ý, kiến thức về tác dụng không mong muốn và tương tác thuốc còn rất thấp (lần lượt là 25,5% và 43,8%). Phân tích hồi quy tuyến tính chưa tìm thấy mối liên quan có ý nghĩa thống kê giữa điểm kiến thức và điểm tuân thủ (p > 0,05).
Kết luận: Mặc dù tỷ lệ tuân thủ cao, nhưng lỗ hổng kiến thức về an toàn thuốc (tác dụng phụ, tương tác) là vấn đề đáng báo động. Các can thiệp giáo dục sức khỏe cần chuyển dịch trọng tâm từ nhắc nhở uống thuốc sang nâng cao hiểu biết về an toàn thuốc và xử trí tác dụng không mong muốn, đặc biệt cần phát huy vai trò của người hỗ trợ điều trị trong gia đình.
