Tóm tắt
Mục tiêu: (1) Mô tả đặc điểm lâm sàng, nội soi và mô bệnh học của polyp đại trực tràng kích thước ≥ 20 mm; (2) Đánh giá hiệu quả và độ an toàn của kỹ thuật kẹp clip dự phòng trong cắt các tổn thương này qua nội soi.
Đối tượng và phương pháp: Nghiên cứu quan sát được thực hiện trên 63 bệnh nhân với 67 polyp đại trực tràng kích thước ≥ 20 mm tại Bệnh viện Gia Đình Đà Nẵng (từ tháng 3/2024 đến tháng 9/2025). Các kỹ thuật can thiệp bao gồm cắt polyp bằng thòng lọng điện (HSP), cắt hớt niêm mạc nội soi thông thường (CEMR) và cắt hớt niêm mạc nội soi có cuống (S-EMR). Tất cả các trường hợp đều được kẹp clip dự phòng đóng kín diện cắt. Các tiêu chí đánh giá chính gồm tỷ lệ cắt nguyên khối, biến chứng sớm/muộn và kết quả nội soi theo dõi sau 3 tháng.
Kết quả: Bệnh nhân chủ yếu là nam giới (55,6%) và trên 40 tuổi (77,8%). Polyp tập trung nhiều nhất ở đại tràng sigma (38,8%) và trực tràng (22,4%). Tỷ lệ cắt nguyên khối chung đạt 83,6%, trong đó kỹ thuật HSP đạt tỷ lệ cao nhất (92,5%, p = 0,01). Tổng cộng có 119 clip đã được sử dụng (trung bình 1,8 ± 1,2 clip/polyp). Đáng chú ý, không ghi nhận trường hợp nào bị chảy máu muộn sau cắt polyp (tỷ lệ 0%). Các biến chứng tức thì (chảy máu 3,2%, thủng 1,6%) đều được xử trí thành công qua nội soi. Sau 3 tháng theo dõi, tỷ lệ sót mô/tái phát là 3,2% và 6,3% trường hợp còn lưu clip cũ.
Kết luận: Kẹp clip dự phòng sau cắt polyp đại trực tràng kích thước lớn (≥ 20 mm) là một chiến lược an toàn và hiệu quả cao trong việc kiểm soát các biến chứng tức thì và ngăn ngừa chảy máu muộn.
