Endovascular treatment outcomes for atherosclerotic femoropopliteal disease in patients with chronic limb-threatening ischemia

Abstract

Background: Chronic limb-threatening ischemia (CLTI) due to atherosclerotic disease is associated with a high risk of major amputation and mortality. Femoropopliteal lesions are often long and multilevel and may be difficult to revascularize, while domestic data on endovascular treatment outcomes remain limited.

Objective: To describe the clinical characteristics, comorbidities, and anatomical features of femoropopliteal lesions and to evaluate the early and 12-month outcomes of endovascular femoropopliteal revascularization in patients with CLTI.

Materials and Methods: This single-center retrospective cohort study consecutively included 36 patients with 43 limbs undergoing endovascular femoropopliteal revascularization at Thong Nhat Hospital from 01/2023 to 01/2025. Baseline characteristics and mortality were analyzed at the patient level, whereas lesion characteristics, technical outcomes, primary patency, and limb salvage were analyzed at the limb level. Outcomes were assessed immediately after the procedure and at 6 and 12 months.

Results: The mean patient age was 74.3 years, chronic kidney disease was 41.7%. Among the 43 treated limbs, tissue loss was 41.9%, multilevel disease in 55.8%, and TASC II C/D lesions in 74.4%. The ABI increased from 0.44 ± 0.11 before intervention to 0.71 ± 0.15 after intervention. Technical success was achieved in 90.7% of limbs, and clinical improvement by at least one Rutherford category was observed in 95.3% after intervention. The observed primary patency rates at 6 and 12 months were 81.4% and 79.1%, respectively. At 12 months, clinical improvement was maintained in 86.0%; four limbs underwent major amputation, corresponding to a crude limb salvage rate of 90.7%. No perioperative deaths were recorded, while 12-month all-cause mortality was 11.1%. Post-contrast acute kidney injury occurred in 25.0% of patients.

Conclusion: Endovascular femoropopliteal revascularization achieved a high technical success rate, improved the ankle–brachial index, and maintained patency in most treated limbs during the 12-month follow-up. The 12-month limb salvage rate was 90.7%. These findings should be interpreted in light of the retrospective, single-center design and small sample size.
