Abstract
Background: laparoscopic sacrohysteropexy is a well-established surgical treatment for uterine prolapse. However, the operative time remains a concern due to technical complexity. Modifications in surgical steps, particularly the timing of peritoneal closure, may improve operative efficiency.
Objective: to evaluate the feasibility, safety, and time-saving efficacy of pre-fixation lateral peritoneal closure in laparoscopic sacrohysteropexy.
Materials and Methods: a comparative cross-sectional study was conducted on 56 women with POP-Q stage ≥ II who underwent laparoscopic sacrohysteropexy at Hue Central Hospital (Vietnam) between October 2023 and April 2025. Patients were assigned to two groups: Group A (n=30) underwent pre-fixation lateral peritoneal closure, and Group B (n=26) underwent the conventional post-fixation closure. Operative time, success rate, complications, and early postoperative outcomes were analyzed.
Results: group A had a significantly shorter operative time than Group B (77.5 ± 18.6 vs. 115.4 ± 23.7 minutes, p < 0.001). The overall success rate of mesh fixation to the sacral promontory was 93.3%, with four failures due to anatomical obstruction by dilated external iliac veins. No intraoperative complications or organ injuries were recorded. At 1-month follow-up, 87.5% of patients were asymptomatic, while 7.1% reported mild pelvic discomfort and 5.4% had mild prolapse recurrence (POP-Q I).
Conclusion: pre-fixation peritoneal closure is a safe and feasible modification that significantly reduces operative time in laparoscopic sacrohysteropexy. This simple technique may enhance surgical ergonomics without increasing complications, supporting its adoption in clinical practice.
