Tóm tắt
Mục tiêu: Định lượng gián tiếp hàm lượng sắt tại các nhân xám trung ương dựa trên hình phase từ chuỗi xung nhạy cảm từ SWI tại Bệnh viện Đại học Y - Dược Huế.
Phương pháp: Nghiên cứu mô tả cắt ngang trên 62 bệnh nhân chưa ghi nhận bất thường trên MRI sọ não có sử dụng chuỗi xung SWI từ tháng 03/2023 đến tháng 04/2024. Độ lệch phase của các nhân xám trung ương được đo lường từ hình phase của chuỗi xung SWI. Nghiên cứu sử dụng thống kê mô tả và kiểm định so sánh cặp theo giới tính và vị trí (trái/phải), tương quan so với tuổi; p < 0,05 được xem là có ý nghĩa thống kê.
Kết quả: Giá trị phase cao nhất được ghi nhận tại đồi thị, tiếp theo là bèo sẫm, nhân đỏ, chất đen, cầu nhạt và nhân đuôi. Tuổi tỉ lệ thuận với mức độ lắng đọng sắt tại nhân đuôi, bèo sẫm, cầu nhạt, đồi thị và chất đen (p < 0,05). Trong đó, bèo sẫm thể hiện mối tương quan mạnh nhất (r > 0,5; p < 0,05). Có sự khác biệt có ý nghĩa về nồng độ sắt giữa hai bán cầu não tại các vị trí bèo sẫm, cầu nhạt và nhân đỏ. Sự khác biệt có ý nghĩa giữa nam và nữ được tìm thấy ở nhân đỏ và cầu nhạt (p < 0,05).
Kết luận: Gia tăng lắng đọng sắt theo độ tuổi tại hầu hết các vị trí, bao gồm nhân đuôi, bèo sẫm, cầu nhạt, đồi thị và chất đen. Sự phân bố sắt cho thấy tính bất đối xứng: nồng độ sắt tập trung cao hơn ở bán cầu phải đối với bèo sẫm, trong khi ở bán cầu trái, sắt lắng đọng nhiều hơn tại cầu nhạt và nhân đỏ.
